A medical examination, also referred to as a clinical examination, check-up, or medical, is the procedure during which a doctor examines a patient's body for indications of disease. Typically, it occurs after gathering the patient's medical history, which includes a description of the symptoms experienced. The physical examination, combined with the medical history, assists in establishing an accurate diagnosis and formulating a treatment plan. All information gathered during this process is documented in the patient's medical record.
