Community Nurses

Community nurses are integral members of the primary healthcare team,
providing essential services to patients in their homes or residential care
settings.
In addition to delivering direct patient care, district nurses also have an
educational role, empowering patients to manage their own care or
instructing family members in caregiving techniques.
Their role is critical in minimizing hospital admissions and readmissions,
facilitating prompt discharge to patients' homes whenever possible.
As a community nurse, you will assess the healthcare needs of patients and
families, ensure the quality of care provided, and maintain professional
accountability for care delivery. Patients under your care may vary in age,
with many being elderly or recently discharged from the hospital, while
others may be terminally ill or have physical disabilities.
Your duties may include daily or multiple daily visits to patients, offering
assistance, guidance, and support. Collaboration with social services,
voluntary agencies, NGOs, and other groups is common to provide and
coordinate a comprehensive range of care services.
The issuance of Registration Certificates, Registration Statements, Badges,
and duplicate badges to community nurses falls under the jurisdiction of the
Malaysia Midwifery Board.
